Forms for LAGOS STATE SCREENING TEST for admission of Primary Six (6) Pupils INTO JUNIOR SECONDARY SCHOOLS (JSS 1) in Model Colleges/Up-graded JUNIOR SECONDARY SCHOOLS with BOARDING FACILITIES for Year 2019/2020 Academic Session. are now ongoing.
- Only Primary Six (Pry 6) pupils in all Public and Private Primary Schools are eligible to participate in Screening Test.

The sales of the forms commenced on Monday, 14th January 2019.
GUIDELINES / METHOD INVOLVED IN PURCHASING THE LAGOS STATE MODEL COLLEGES ADMISSION FORMS 2019/2020
Parents who are interested in gaining admission for their wards into any of the Lagos State Model Colleges/Upgraded Junior Secondary Schools are advised to purchase the admission forms through the Lagos State Examination Board.
Admission Forms of the Lagos State Model Colleges/Upgraded Junior Secondary Schools cost N10,000. Payments are only to be made through the accounts of the Lagos State Federal Ministry of Education.
Parents, Guardians, Head Teachers and Proprietors can pay the sum of Ten Thousand Naira (N10,000) only to any of the following Accounts Department of the Lagos State Examinations Board: Skye, Fidelity and First City Monument Bank.
Registration can also be done through a customized compact disc that would be issued by the state examination board after paying N10,000 for the form.
Late registration, would attract an additional fee of N15,000 (Fifteen Thousand Naira) each and would commence on Wednesday 29th April and end on Friday, 24th May 2019.
LAGOS STATE MODEL COLLEGES SCREENING TEST VERSION 2019
Applicants are to note that there will be Two Version of Screening Test to be adopted namely:
(a) COMPUTER BASED TESTING (C.B.T.) ,
(b) PAPER PENCIL TEST (P.P.T.)
The screening would be conducted through Computer-Based Test and the regular Pencil-Based Test, which is the usual pencil and computer answer sheet test.
Candidates were at liberty to choose the version of the test they wished to be examined with.
NB: Candidate that pass the Test and meet the required cut-off marks could be offered admission into any of the Model Colleges/Upgraded Junior Secondary Schools irrespective of his/her first or second choice of schools.
The Entrance Examination/Screening Test Exercise Date has been announced.
